In order to achieve this goal, we need your assistance and understanding
of our payment policy.
The fee for root canal treatment in this
office is based upon the complexity of the tooth which is to
be treated. All fees will be quoted prior to the start of treatment.
It is the policy of this office that the full fee be paid
at the completion of treatment. For your convenience we accept cash,
checks, MasterCard, Visa, or Discover. Additionally, we have partnered
with the following finance companies to offer our patients flexible
payment plans to fit individual needs and financial circumstances:

In cases where an insurance company
covers the cost of treatment we will gladly submit the claim to your insurance
company, provided that you have supplied us with the required
information. You must realize that your insurance company has
a contract between you, your employer and the insurance company.
We are not a party to that contract. You are responsible for
payment regardless of any insurance company's arbitrary determination
of Usual and Customary rates. Most insurance companies do not
cover 100% of the charges; we do require an estimated deposit
on completion of treatment. Please refer to the insurance information
